Learning Disability Social Worker, Redbridge To provide high-quality social work services to adults with learning disabilities, ensuring their rights, independence, and well-being are promoted. The role involves working collaboratively with individuals, families, and multi-disciplinary teams to deliver person-centred care and support.
Key Responsibilities
Assessment & Care Planning
Undertake comprehensive assessments of individuals with learning disabilities in line with statutory requirements.
Develop and implement person-centred care plans that promote independence and choice.
Safeguarding
Identify and manage safeguarding concerns, ensuring compliance with local and national safeguarding procedures.
Case Management
Manage a complex caseload, prioritising effectively and meeting deadlines.
Maintain accurate and timely records in accordance with professional standards.
Multi-Agency Working
Work collaboratively with health professionals, housing, education, and voluntary organisations to deliver integrated support.
Advocacy
Promote the rights and voice of individuals with learning disabilities, ensuring they are involved in decisions about their care.
Legal Compliance
Apply relevant legislation including Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
Professional Development
Participate in supervision, training, and continuous professional development activities.
